Treatment Packages and Pricing

Office Consultation - 90 minutes


Typical first visit to establish and obtain history for chronic ailments.  Patient will receive a detailed plan and resources by email within 24 hours of the appointment.

Office Consultation - 60 minutes

$ 225

Best for established patients working on a few chronic issues.

Office consultation - 45 minutes


Appropriate for follow up visit.  Can be completed as a video or phone consultation.

Brief Visit/Focused Phone Consult- 20 minutes

$85 - Available to established patients only

  • For focused problem of short-term bladder infection, respiratory infection, stomach bug.  

  • For focused follow up on previous visit

Mini-Integrative Package

$860 plus any lab fees.  

For moderate problems that are not responding satisfactorily to the current therapies.

Examples:  Migraine headaches, Eczema, chronic constipation or bloating, recurrent vaginitis

  • 3 office visits with Dr. Jain (initial consultation and exam, 2 in-person follow-ups)

  • 5 email follow ups/check-ins

  • Assessment and plan notes emailed directly to the patient

  • Curated online reference material appropriate to patient’s situation

Complex Integrative Medicine Package


For complex, chronic states of feeling unwell and over-prescribed.

Examples: Fibromyalgia, Mild Memory loss, Fatigue,  Irritable Bowel Syndrome/Bloating/Constipation, Insomnia, Metabolic syndrome, Obesity/overweight, Fatty Liver Disease, Autoimmune disorders.  

  • 4 Visits with Dr. Jain (110 minute initial consult and exam, 3 follow-up visits)

  • Weekly phone or email follow ups/check ins for the length of the treatment plan.  Expect the course of treatment to take 4-6 months.  

  • Curated online reference material appropriate to patient’s situation

* Important Info

This is not an insurance-based practice.  All payments will be made at the time of the service.  A bill will be provided with and ICD-10 code and CPT code that can be submitted to your insurance, but reimbursement will be up to your insurance carrier.  Typically, Dr. Jain's visits are covered as an out-of-network provider. 

Medicare and Medicaid do not cover these services.

Dr. Jain will not prescribe narcotics, stimulants, benzodiazepines or other controlled substances.  This practice de-emphasizes prescriptions of all kinds.
